Universal Permission Form -
Procedure
A new universal
Annual Permission form has been released and is to be used beginning this
year—Sept 2007.

website for a copy that may be downloaded and printed.

1) One form now
covers: trips, product sales participation, photo release,
emergency medical treatment, and health release.
2) Every girl in a
troop needs to fill one out and return it to the leader who keeps the packet
with her for use during year. Forms should be shredded at end of year.
3) Leader is to make a
photo copy of each troop members form (an emergency copy to be kept with SU
files); clip together; give packet to School Coordinator.
4) School Coordinator
gives to Leader Support Manager to keep with SU 1 files at the next Leader
Meeting.
5) It is still
important to create for each event a separate Event Permission Form for
each girl that names and explains the activity, where held, cost, depart
time, return time, telephone number of lead chaperone during event, and
parent emergency contact number for that date.
6) Both forms should
accompany the event.
helpful hint to
leaders: place the
universal Annual Permission form for each girl in a transparent sleeve for use
during the year; you can then place the completed event permission form in the
sleeve for that event and then you have everything you need for each girl for
that event!
